General Enrollment Information
As a public charter school, any child residing within Licking County may enroll at Par Excellence STEM Academy. If you want to inquire about new student enrollment, please feel free to call our staff at any time by calling 740-344-7279, or send an email at kejohnson@parstem.org. We enroll all grades from Kindergarten through 6th grade.
If you are interested in enrolling your child, follow the instructions above to access our Final Forms student registration portal.
The following documentation MUST be included for enrollment to be complete:
- Birth certificate (Must have original for office to copy)
- Immunization record
- Photo I.D. of custodial parent or guardian
- Proof of residency (Must be within the past 30 days and have a parent/guardian name and current address) Examples include: utility bill, lease, mortgage, etc.
- Proof of custody (In cases of divorce or foster families)
- Current IEP or ETR Plan (If applicable)
- The completed Final Forms New Student Registration application, which includes:
- Consent for Student Records Release
- Language Usage Survey
- Bus Transportation Registration
